Crohn's Food Diary Handout in English & Spanish

This Patient Education Handout helps people experiencing Crohn's Disease keep track of foods that they can digest well and foods that cause them pain after eating. This one sheet split diagram is meant to be a simple resourceful tool and not a multi-page workbook. this is a pre-structured diary that helps organize someone's experiences with food more effectively than a generic list on a spare sheet of paper or on a digital note in a mobile device.
Nutrition Enthusiasts with Crohn's can also download this sheet for free in English/Spanish without receiving it first at a medical appointment. Someone can write on the PDF using a computer or using the photo's app in their mobile device by taking a screenshot of the free PDF while visiting the Clinical Blog on their mobile phone. Someone does not need access to a printer to be able to use this handout. the handout can be re-downloaded or digitally re-printed to PDF more than once from this blog post if an individual needs more than one page to record all their Crohn's Food Preferences.
Digital Convenance :
The benefit of being able to use this sheet on a mobile device is that someone can directly and quickly record if they have a bad reaction to food while in a restaurant, networking at a public event, at work in a private office, out with friends, or on vacation. By taking a screenshot of the PDF and saving the picture to its own unique album on an individual's phone called "Crohn's Food Diary" the individual is creating their own digital disease management network that is the most convenient way to keep track of Crohn's experiences. When a bad food reaction happens the individual just adds text to the picture in a photo app's "Edit" tool directly into the "Foods that Cause Pain" section of the sheet on their mobile device. If someone ends up using all the space on the sheet, then they can visit the Nonprofit's blog again and take another screenshot of the free PDF.
Whether a patient's preference is digitized information or handwritten notes, this free handout is adaptable to someone's preferred method of notetaking.
