This free handout is designed for streamlined communication between patient/client & provider so that the patient can easily circle which foods would be health promotional to add into their weekly diet. This handout can be used in accompany with motivational interviewing and cognitive behavioral therapy during inpatient or outpatient nutrition education. This handout is designed to help increase the intake of lean protein, fresh fruits/vegetables, complex carbohydrates, whole grains, & health promotional cooking oils (labeled as healthy fats). This handout is designed to help patients/clients who dislike handwriting grocery lists remember which foods they would like to incorporate in their diet. This free handout is available in both English/Spanish.
This handout works well for Inpatient Nutrition Education that is being utilized as part of Documented Nutrition Intervention for an Inpatient Nutrition Diagnosis to help ease the patient into participating in discharge planning. In Outpatient MNT (Medical Nutrition Therapy) this handout can be used to help patients who are in the: Contemplation, Preparation, Action, & Maintenance phases of the Transtheoretical Model of Behavior Change.
